Certificate of Studies for Previous School Years (Academic Grades, Conduct Grade, Disciplinary Records)

  • Birth certificate
  • Parents ID’s
  • Marriage/divorce certificate

ORIGINAL

  • Student School Transcript
  • Medical history form filled by a family doctor (school format)
  • Immunization records / the student vaccination
  • 4 recent passport-size photos.
